Click HERE to visit the LHSAA official site The following LHSAA forms are required for students to participate in PJP II Sports (available from the school office and coaches). YOU MUST HAVE ALL 4 COMPONENTS TO BE ELIGIBLE TO PARTICIPATE IN LHSAA SPORTS
- LHSAA Athletic Participation / Parental Permission parts I & II must be completed each school year, prior to first practice date.
- Medical History form must be completed each school year, prior to first practice date.
- LHSAA Substance Abuse / Misuse Contract and Consent must be completed prior to participating in first sport at PJP each year.
- Birth Certificate: a copy must be on file in the school office.
